Job Summary:
 
The Nurse Practitioner functions as an independent healthcare provider with an advanced degree of knowledge, skill, and expertise in history taking, physical exams, health assessments, and promoting healthy outcomes of members. This is a travel assignment working from a home-based office and meeting with members, primarily in their homes.  The NP will conduct the initial health assessment of new and existing members, document their findings, and transfer the completed medical assessment to a care management team that will design and implement a care plan for each member.  The NP will strictly be responsible for the initial health assessment of members, not the long term treatment or care management of members.  The Nurse Practitioner will primarily interact with members of the geriatric population in their homes, with a typical visit lasting around 1 hour.
 
Position Description:
 
The Nurse Practitioner performs physical examinations and takes medical history reports in order to thoroughly and accurately documents the member’s current medical conditions and overall health state.  Orders, interprets, and evaluates diagnostic and therapeutic procedures to identify and assess patient’s clinical problems and health care needs, explaining the necessity, preparation, nature and anticipated effects of scheduled diagnostic to the patient.  Takes patient histories, conducts physical examinations, records physical findings, and prepares patient workups and summaries noting pertinent positive and negative findings.  Records assessment and any related recommendations or documents in the members Electronic Medical Record (EMR).  The Nurse Practitioner discusses each case with the Physician and other health professionals to assist in preparing a comprehensive patient care plan.  Provides recommendations for diagnostics, treatments, or medications based on initial health assessment.  Refers patients to the Physician for consultation or specialized health resources for treatment.  Ensures complete and accurate hand-off of member medical records and any relevant recommendations to the designated care management team.
 
Background Profile:
 
Valid, unrestricted state Nurse Practitioner (N.P.) certification and Advanced Practice Registered Nurse (A.P.R.N.) license.
Requires current and valid CPR certification.
Requires 1-2 year experience with geriatric population as an RN or NP.
Intermediate computer literacy, including electronic medical record documentation, is required.
Prefer candidates certified to work with the adult population age group, or experienced at home care / home visits. 
Candidate should have excellent people skills, with superior written and oral communication skills, and the ability to work both as a team member and independently. 
Valid State Driver's license and vehicle for travel.
Must be willing and able to travel in area (mileage reimbursed).
