Disease Management Nurse Job Summary:
  • Disease Management Nurse / Health Coach Nurse plays an integral part in monitoring and assessing ongoing member health risks in real time and providing members with the tools and resources necessary to better manage medical conditions and make behavioral changes.
  • The primary responsibility of the Disease Management Nurse is to coordinate and monitor the care of Health Plan members and develop a nursing plan of care.
  • They will participate in interdisciplinary conferences to review clinical assessments, update care plans and determine follow-up frequency.
  • The Disease Management Nurse also acts as an advocate for members and their families linking them to other members of the care team.
  • The Disease Management Nurse helps them gain knowledge of their disease process and to identify community resources for continued growth toward a maximum level of independence.
Disease Management Nurse Background:
  • Valid, unrestricted state Registered Nurses (R.N.) license and Certification in Case Management (CCM) is required.
  • Requires Bachelor of Science or Arts; can be in any field of study.
  • Roughly 2-3 years acute care clinical nursing experience, including experience with chronic or complex conditions, or multiple comorbidities.
  • Roughly 5-6 years’ experience in Preventative Health, Case Management, or Disease Management. Wellness or Health Coaching experience is strongly preferred.
  • Around 1-3 years’ experience in a Managed Care Organization (Health Plan/HMO/MSO/TPA/IPA/etc) is a plus.
  • Experience working with Federal and State assistance programs (Medicare/Medicaid/etc) Computer literacy (MS Office) and typing skills are necessary.
  • Strong oral and written communication skills; ability to interact within all levels of the organization as well as with external contacts.
