Manager of Field Case Management Job Summary: 
  • Provides ongoing consultation to Field Service Coordinators and reviews caseloads on an ongoing basis, both through regular face-to-face supervision sessions and review of management reports and deadlines.
  • Ensures the case management process of assessing, planning, implementation, coordination, monitoring, and evaluating services and outcomes is pursued to maximize the health of the member.
  • Promotes effective healthcare utilization, monitors health care resources and assumes a leadership role to achieve optimal clinical and resource outcomes for member.
  • Oversees the socioeconomic needs and services of selected member populations across the continuum of illness. 
  • Develops and implements case management programs and strategies, as well as workflows, policies, and procedures.
  • Perform audits of assessments, care plans and service notes to verify cases are properly established and that member coordination activities are occurring and appropriately documented.
  • Coordinates activities of unit to meet contractual company deadlines and resolves interpersonal conflicts.
  • Partners & collaborates with other departments cross functionally regarding care and case management and/or Health Service initiatives.
  • Directs work assignments, measures results and initiates personnel actions as required.
Manager of Field Case Management Background: 
  • Valid, unrestricted state Registered Nurse (R.N.) license
  • Bachelor's Degree in Nursing or Health Administration is preferred. Certified Case Manager (CCM) preferred.
  • 5 years of acute care clinical nursing experience.
  • 3-5 years’ Case Management experience;
  • 3 years’ experience at a health plan or other managed care organization (HMO/TPA/IPA/etc).
  • Familiarity with Medicaid/Medicare and state managed care practices, programs, and policies.
  • Preferred Other NCQA and/or EQRO accreditation experience
  • Experience in training / teaching staffs to meet operational requirements and goals.
  • Strong oral and written communication skills; ability to interact with all levels of internal and external contacts.
