Your Job Summary:

The Nurse Practitioner (N.P.) functions as an independent healthcare provider with an advanced degree of knowledge, skill and expertise in history taking, physical exam, treatment and clinical care management of members. The NP or PA will be interacting with new and existing members at the Company's Care Centers, and other care environments.  The NP or PA serves patients with both acute and chronic conditions and provides exceptional care to members by providing diagnosis, treatment, consultation, and follow-up under the supervision of one or more physicians.  In this full-time position, they will work in conjunction with Physicians to provide care and care management to this group of patients. The NP will primarily interact with members at established clinics and company care centers.
Your Background:

Valid, unrestricted state Nurse Practitioner (N.P.) certification and Advanced Practice Registered Nurse (A.P.R.N.) license.
Furnishing and DEA licensure in good standing with the state.
Requires 2-4 year experience as an direct care provider, preferably in an ICU/ER or other fast-paced stetting. 
Experience working with geriatric populations is a plus.
Intermediate computer literacy, including electronic medical record documentation, is required.
Prefer direct experience working with the NextGen EMR platform.
The candidate must be certified to work with the adult population age group.
Prior experience in a managed care organization and/or a clear understanding of managed care systems preferred.
Excellent assessment skills required; able to problem-solve with an awareness of level of care, intensity of service and severity of illness criteria. 
Excellent patient and family communication skills (written and oral) and a sensitivity to different cultures.
Ability to work effectively and collaboratively with a variety of individuals including physicians, health plans, hospital and provider office staff, patients and their families.
Bilingual Spanish speaker strongly preferred.
Your Duties:

The Nurse Practitioner assumes responsibility for developing therapeutic relationships with patients, and formulates a care plan and prognosis, based on the patient’s condition.  The NP or PA acts as the key point of contact for patients managing both acute and chronic diseases and advises patients on goal setting and action plans as the primary focus of managing their disease.  They perform physical examinations and preventive health measures within prescribed guidelines in order to assist physician in diagnosing medical problems and prescribing necessary treatment and services for quality patient care.  Orders, interprets, and evaluates diagnostic and therapeutic procedures to identify and assess patient’s clinical problems and health care needs, explaining the necessity, preparation, nature and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient.  Takes patient histories, conducts physical examinations, records physical findings, and prepares patient workups and summaries noting pertinent positive and negative findings.  The Nurse Practitioner discusses each case with the Physician and other health professionals to prepare a comprehensive patient care plan.  Submits the health care plan and goals of individual patients for periodic review and evaluation by the Physician, consulting with supervising Physician when patient's progress does not meet anticipated or predetermined criteria.  Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures.  Refers patients to the Physician for consultation or specialized health resources for treatment.  The NP works directly with patients and their caregivers to monitor their care, to identify and facilitate completion of preventative and follow-up care as it related to assigned patients, and to conduct home visits at the patient’s home when needed.
