Social Worker Job Summary:
  • The Social Worker is responsible for care coordination and family health education with identified Medicaid members.
  • Develops and manages an individualized intervention plan for members with the goal of promoting optimal, achievable outcomes in the most cost effective and appropriate manner.
  • Performs comprehensive assessment of physical, emotional, psychosocial, and environmental needs of the member by reviewing relevant and available medical records and communicating by telephone and in person.
  • They provide timely and effective social work care planning that encompasses initial and continuing psychosocial assessment of individuals for their medical, financial, emotional, and spiritual needs.
  • Primary goals include closing gaps in preventive care, addressing barriers and supporting the member/family self-management of chronic illness. 
 
Social Worker Background:
 
  • Requires a valid, unrestricted state as a Licensed Bachelor’s Social Worker (L.B.S.W) or Licensed Master’s Social Worker (L.M.S.W.).
  • Mental Health Professional required as defined by State: Master degree in social work, behavioral health or related field, and 2 additional year’s documented and supervised experience.
  • Prefer 3+ years of acute care clinical social work experience; including 2 years working with a pediatric/adolescent population.
  • Strong working knowledge of care planning/case management procedures, and familiarity with area mental health and substance abuse community resources.
  • Intermediate computer literacy (MS Office) and typing skills are necessary.
