Community Resource Specialist (LCSW/LMSW or RN/LPN) – Health Plan
Atlanta, GA (Northwest Suburb, near Smyrna, GA)

Community Resource Specialist Job Summary: 
  • Conduct screening and risk assessment interventions per program guidelines
  • Assist with non-medical aspects of the member’s care, including referrals to community resources
  • Identify special needs members through the completion of health screens and other resources
  • Work with community outreach/member advocates to coordinate member care
  • Educate providers and community resources on program components and available support services
  • Educate members with special needs to foster compliance with program and positively impact outcomes
  • Conduct site visits as appropriate for programs and provide support to other special programs
  • Develop and modify care plans in conjunction with member, member’s family and managing physician
  • Development of plan specific literature and education materials in conjunction with medical director and corporate oversight
Community Resource Specialist Background: 
  • Requires a valid, unrestricted state license as one of the following:
    • Licensed Master Social Worker (LMSW)
    • Licensed Clinical Social Worker (LCSW).
    • Registered Nurse (RN) with a behavioral health background
    • Licensed Practical Nurse (LPN) with a behavioral health background
  • Position does NOT require Bachelor’s or Master’s degree (unless required to receive state license)
  • Requires 2 years of experience in managing and integrating the social/community needs of members
  • Established experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs is required
  • Prefer Behavioral Health / Mental Health Case Management experience from a Health Plan, IPA, HMO, or other Managed Care setting
  • Intermediate computer literacy (MS Office) and typing skills are necessary
  • Strong oral and written communication skills; ability to interact within all levels of the organization as well as with external contacts
  • Must be able to prioritize, plan, and handle multiple tasks/demands simultaneously
  • Bilingual skills (Spanish/English) is preferred
  • FIELD POSITION: Must be willing and able to travel throughout the assigned area (mileage reimbursed).
