Behavioral Health Professional Job Summary:
The Behavioral Health Case Manager plays an integral part in the coordination of care assessing the member’s condition and providing them the necessary resources, options, and coordination for a healthy care outcome.
The primary responsibility of the Nurse Case Manager is to coordinate and monitor the care of Health Plan members and develop a plan of care.
The Behavioral Health Case Manager also acts as an advocate for members and their families linking them to other members of the care team to help them gain knowledge of their disease process and to identify community resources for continued growth toward a maximum level of independence.
Behavioral Health Professional Background:
Requires a master's degree prepared individual with valid and unrestricted state licensure in Behavioral Health, such as:
Licensed Associate Counselor (L.A.C)
Licensed Professional Counselor (L.P.C)
Licensed Clinical Social Work (L.C.S.W.)
Licensed Master Social Work (L.M.S.W.)
Will consider Licensed Associate Substance Abuse Counselor (L.A.S.A.C.) and/or Licensed Independent Substance Abuse Counselor (L.I.S.A.C.) only in conjunction with well-rounded crisis experience
Requires 1-2 years of experience with facility-based and/or outpatient psychiatric and substance abuse or substance abuse disorder treatment.
Requires strong working knowledge of utilization review procedures, and familiarity with area mental health community resources.
Intermediate computer literacy (MS Office) and typing skills are necessary.
Strong oral and written communication skills; ability to interact within all levels of the organization as well as with external contacts.
Must be able to prioritize, plan, and handle multiple tasks/demands simultaneously.
