  • Licensed Clinical Social Work (L.C.S.W.)
  • Licensed Professional Counselor (L.P.C.)
  • Licensed Marriage and Family Therapist (L.M.F.T.)
  • Licensed Mental Health Counselor (L.M.H.C.)
  • BH/Psych experienced Registered Nurse (R.N.) 
 
Case Manager Job Summary:
  • Responsible for performing telephonic case management within the scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs.
  • Uses appropriate screening criteria, knowledge and clinical judgment to assess member needs.
  • Conducts assessments to identify individual needs and develops care plan to address objectives and goals as identified during assessment.
  • Monitors and evaluates effectiveness of care plan and modifies plan as needed.
  • Supports member access to appropriate quality and cost effective care.
  • Coordinates with internal and external resources to meet identified needs of the members and collaborates with providers.

Case Manager Background: 
  • Requires a master's degree prepared individual with valid and unrestricted state licensure and established behavioral health experience.as a:
    • Licensed Clinical Social Work (L.C.S.W.)
    • Licensed Professional Counselor (L.P.C.)
    • Licensed Marriage and Family Therapist (L.M.F.T.)
    • Licensed Mental Health Counselor (L.M.H.C.)
  • Will consider Registered Nurses (R.N.) that have extensive psychiatric/ behavioral health case management experience.
  • Certification in Case Management (CCM) is preferred.
  • At least 3-5 years Behavioral Health / Mental Health Case Management experience.
  • At least 1 year of experience in a Managed Care Organization (Health Plan/HMO/MSO/TPA/IPA/etc).
  • Experience working with Federal and State assistance programs (Medicare/Medicaid/etc) is preferred.
  • Basic computer literacy (MS Office) and typing skills are necessary.
