Job Summary:
Plan, implement and evaluate nursing care to achieve objectives of the agency:
Assign and coordinate patient care ensuring appropriate skill level of staff meets needs of patients; ensure staff compliance with maintenance of Patient’s Rights, Advanced Directives and ethics; ensure staff compliance with disaster preparedness for field and office staff, safety regulations and universal precautions. Prompt, knowledgeable, courteous service
Support the coordination of interdisciplinary care:
Assist with coordination of care between the inpatient and home and collaboration among disciplines within the home care department; appropriately triages incoming phone calls, interact extensively with physicians, patients, families and all levels of staff, both in the agency in the hospital and in the community
Always functions as a patient advocate and clinical role:
Exercise cultural sensitivity when dealing with a diverse patient, family and guest population in regards to sex, race, religion, age, sexual orientation, marital status culture, etc. Provide and contribute to a positive working environment to implement and reflect the mission statement of the organization through the Core Values
Documentation/Information Management:
Observe, evaluate and report to physician patient’s physical and emotional condition, reaction to drugs or treatments, interpret to MD social and physical in the environment that effect patient’s care and convey information to team members and transcribes MD supplementary orders as needed:
Ensure compliance with staff members to provide organizational and patient confidentiality
Review patient initial and on going Plan of Treatment, recertification and discharge summaries to ensure compliance with agency policy, law and regulations
Leadership/Supervision: up to 100 FTEs
Function autonomously, utilizing appropriate decision making processes with awareness of when to ask for assistance from Clinical Director
Demonstrate effective problem solving skills and follow through on patient care issues including notification of appropriate personnel, physician and collaborating with the Clinical Supervisor or Director of Patient Care Services as needed
Hire, schedule and manage the performance of the assigned team, including nurses, social workers, chaplains, home health aides, and other clinical disciplines
Conduct all interactions with staff in a firm, fair, consistent and objective manner. Promptly and fully investigate and resolve complaints of patients, families and physicians through own action or referral to a higher authority
Demonstrate an ability to respond and provide direction in changing workload during crisis situations and/or in a rapidly changing work environment
Development:
Maintain current awareness of new legislation, regulation and The Joint Commission standards and ensure compliance with State and Federal regulations, both within the realm of own job responsibilities and those of staff development
Assume responsibility and accountability for own professional growth and practice. This includes maintaining current licensure and completing all required in-service education
Attend all assigned staff development activities for department including a minimum attendance of 75% at all scheduled work time staff meetings
Participate in/conducts in-service education, as requested

Preferred Background:
California RN license
BLS certification
Current CA Driver license
Automobile insurance
Minimum 1-year Home Care experience
Strong verbal and written communication skills
Able to interact effectively with all levels of health care personnel
